Waiver

Participation Waiver, Assumption of Risk & Consent (Trial Classes)

Completion of Waiver
All Participants must complete this waiver before attending any trial classes organised by Gyodokan West London Aikido (the “Activities”). Where the Participant is under 18 years of age, this waiver must be agreed to by a parent or legal guardian. By ticking the relevant box, the parent or legal guardian confirms that they are authorised to provide consent on the Participant’s behalf.
Confidentiality
All information provided is confidential and will not be shared with any third party except where required for safeguarding, medical, insurance, or legal reasons.
Acknowledgement of Inherent Risk
The Participant acknowledges that the Activities involve inherent risks, including but not limited to physical injury, accidents, falls, physical contact, joint locks, throws, and other techniques associated with the practice of aikido, as well as the use of shared training spaces and equipment.
Communicable Illness Risk
The Participant understands that participation in group physical activities in shared indoor environments may involve exposure to communicable illnesses or infections through close contact with others or shared facilities or equipment.
Health Declaration & Non-Attendance While Ill
By attending and participating in the Activities, the Participant (or their parent or legal guardian) confirms that, to the best of their knowledge, the Participant is in suitable physical and mental health to train and does not knowingly present an increased risk of transmitting illness to others. The Participant agrees not to attend training while experiencing symptoms of a contagious illness.
Compliance with Safety Rules
The Participant agrees to follow all dojo etiquette, hygiene practices, safety instructions, and reasonable guidelines provided by instructors at all times, and to act responsibly and considerately towards others.
Assumption of Risk
The Participant voluntarily accepts and assumes all risks associated with participation in the Activities.
Medical Responsibility & Disclosure
The Participant (or their parent or legal guardian) acknowledges responsibility for assessing any physical, psychological, or medical conditions that may affect safe participation and confirms that any relevant medical information has been disclosed to Gyodokan West London Aikido.
Emergency Medical Treatment
In the event of injury or illness during the Activities, the Participant (or their parent or legal guardian) authorises Gyodokan West London Aikido and its representatives to obtain or administer emergency medical treatment where reasonably necessary. Any associated costs remain the responsibility of the Participant or their parent or legal guardian.
Waiver & Release of Liability
The Participant, or the Participant’s parent or legal guardian where applicable, agrees to waive, release, and hold harmless Gyodokan West London Aikido, including its officers, agents, employees, organisers, instructors, teachers, representatives, and successors (the “Parties”), from any and all liability, claims, demands, or causes of action arising out of or in connection with participation in the Activities, except where such liability cannot be excluded by law.

IMAGE CONSENT: Photographs and/or short video recordings may occasionally be taken during Aikido classes or events at Gyodokan West London Aikido to show Aikido training and the dojo community. Images are used respectfully and in line with our safeguarding practices. Children are not named or personally identified. *I consent to images of my child being used on the dojo’s website to show Aikido training and activities.I also consent to images being used on the dojo’s social media pages or printed materials to promote Aikido Gyodokan.I do not consent to any images of my child being used.
